Driver Declaration & Consent

This declaration and consent form applies to all drivers participating in any training, assessment, or aftercare service delivered by DriveSmart Solutions.

1. Driver Declaration

I confirm that:

• I hold a valid driving licence for the category of vehicle I will be operating.

• I am legally entitled to drive the vehicle being used.

• I am fit to drive, including but not limited to:

  • Not being impaired by fatigue

  • Not being under the influence of alcohol, drugs, or any medication (including prescription medication) that may affect my ability to drive safely

  • Meeting eyesight and medical requirements

• I understand that I remain fully responsible for all driving decisions while in control of the vehicle.

• I will comply with all lawful instructions provided during training or assessment.

• I will inform the assessor immediately if I feel distracted, unwell, fatigued, or unsafe to continue driving.

2. Nature of Assessment

I understand and acknowledge that:

• The service provided by DriveSmart Solutions is not a driving test.

• The assessment is observational and advisory in nature.

• Outcomes, scores, and feedback represent professional opinion at the time of assessment.

• Assessment results do not guarantee future performance or incident prevention.

3. Safety Authority

I understand that the DriveSmart Solutions assessor may:

• Pause, terminate, or abandon an assessment if safety concerns arise

• Instruct me to stop driving where necessary

• Take control of the vehicle if required to reduce immediate risk

Such actions are taken solely in the interest of safety.

4. Data Protection & Consent

I consent to DriveSmart Solutions:

• Collecting and processing my personal data for the purposes of training, assessment, reporting, and aftercare

• Recording assessment outcomes, observations, and recommendations

• Sharing relevant assessment information with my employer or contracting organisation

I understand that my personal data will be processed in accordance with the DriveSmart Solutions Privacy Policy and UK GDPR.
